To ensure a smooth process, we have prepared instructions on how to properly submit a termination.
Termination form – document
According to the agreement and the Dealavo regulations (Section V, Term of the Agreement, Point 2), the termination must be submitted in writing and signed by a person authorized to represent the company.
What should the termination document include?
- Date of the document
- Agreement number and date of signing
- Details of the terminating company (name, address, VAT number)
- Dealavo’s details
- Statement of contract termination along with the termination date
- Signature of the authorized person
Tip: Make sure that the termination document includes all the necessary information to avoid delays in the agreement termination process.
Ways to submit the termination document
The signed termination document can be sent to Dealavo in one of the following ways:
- Email – A scan or photo of the signed document can be sent to: support@dealavo.com
- Traditional mail – The original document can be sent to Dealavo’s registered office:
Dealavo sp. z o.o.
ul. Włodarzewska 33 lok. 6
02-384 Warsaw, Poland
When does the notice period begin?
The notice period begins on the first day of the month following the month in which we received the signed termination document.
Tip: To avoid unnecessary extensions of the agreement duration, it is recommended to send the termination document as early as possible within the given month.
